Who can refer to our SDS service?

ANYONE can refer to our service:

• The person looking for information about or to apply for SDS

• A friend or relative

• A carer or someone working for social/health care

• Any other relevant service.

Please ensure that if you are referring someone to our SDS service that you have discussed this with them first and they are aware of the referral.
